Which medication route undergo first pass metabolism in the liver?

I'm a pharmacology student, trying to understand drug metabolism better. I need to know which medication routes typically undergo significant first-pass metabolism in the liver. Can someone explain?

Generally, medicines taken orally, like tablets or syrups, go through first-pass metabolism in the liver. This means a good portion of the drug gets broken down before it reaches general circulation. That's why sometimes the same drug needs a higher dose orally than if given by injection.

Achha, first pass metabolism? My liver is doing so much work already just breaking down all the chai and pakoras, now medicines also? Haha! But seriously, if you swallow it, chances are liver will filter it first. All these medical terms make my head spin!

Well, when you swallow a medicine, it usually goes through the liver first. That's why for some things, people prefer ayurvedic or natural remedies, thinking they might be gentler. But for serious conditions, always trust your doctor and their prescribed route, they know best.

So if it goes through the liver first, does that mean the medicine is less effective sometimes? Or that it puts more strain on the liver? I always worry about side effects. Has anyone else felt concerned about this with their meds?

This sounds like something I learned in college but completely forgot after my first all-nighter. I just take whatever the doc says, hoping my liver is strong enough to handle its 'first pass' duties. My brain certainly isn't passing any tests right now!

The oral route is where first-pass metabolism usually happens. Knowing this makes me think about how much 'active' medicine actually reaches the bloodstream. It also makes you wonder about the cost-effectiveness if a big portion is metabolized away. Sometimes injectable forms, though more expensive, might be more efficient for certain drugs, worth discussing with the doc.
